This form is long but helps make sure all aspects of the night are taken care of so that you can spend the night dancing and greeting guests! If you aren't sure about anything, just leave it blank for now, but please complete the first two sections as best you can. You may email me anytime with changes or re-submit this form with new information. I will do my best to fulfill last-minute requests but please try to finalize your decisions at least a few weeks before the day of the wedding.

Once I receive this form, I will mail you a contract which outlines the services I provide and also contact you about pricing if we haven't already discussed it. Please don't hesitate to ask me any questions - I am available starting right now and up until the last dance.

I will also arrange for an in-person meeting about two weeks before your wedding, so that we can finalize the schedule and answer any last-minute questions you might have!
